Depression Risk Assessment
Validated PHQ-2 quick screen with optional full PHQ-9. Over the last 2 weeks, how often have you been bothered by the following problems?
Important: This is a screening tool, not a diagnosis. Results do not replace evaluation by a qualified mental health professional. If you are in crisis or having thoughts of self-harm, please contact a helpline immediately (in India: iCall 9152987821, Vandrevala Foundation 1860-2662-345, or KIRAN 1800-599-0019).
What your score means
PHQ-2: 0-2 — Screen negative. The short screen does not suggest depressive symptoms; the full nine items are not required.
PHQ-2: 3-6 — Screen positive. The short screen is positive, so the full PHQ-9 is offered to measure severity.
0-4 — Minimal. Few or no depressive symptoms reported for the last two weeks.
5-9 — Mild. Some symptoms present. Watchful waiting and a repeat check are usually suggested.
10-14 — Moderate. Symptoms at a level where discussing treatment options with a doctor is appropriate.
15-19 — Moderately severe. Symptoms likely to affect daily functioning. Clinical assessment is recommended.
20-27 — Severe. A high symptom burden. Please arrange prompt review with a qualified doctor.
Frequently asked questions
Is the PHQ-9 a diagnosis of depression?
No. The PHQ-9 is a screening questionnaire that indicates how severe your reported symptoms are. Only a qualified clinician can diagnose depression, after a proper assessment.
What should I do if I scored high, or answered yes to the self-harm question?
Speak to a qualified doctor or mental health professional as soon as you can. Any response other than "Not at all" on the self-harm item deserves a conversation with a clinician, whatever your total score. If you are in immediate danger, contact emergency services or a crisis helpline.
How often should I retake this?
The questions cover the last two weeks, so retaking it more often than every two weeks adds little. Many people repeat it every two to four weeks to follow a trend.
