Geriatric Depression Scale (GDS-15)

15-item depression screen designed for adults aged 60+.

Eligibility: 60–120 years. Enter your age to open the questionnaire.

The GDS-15 is the 15-item short form of the Geriatric Depression Scale developed by Yesavage and colleagues. It uses simple yes or no questions about mood, interest and outlook over the past week, written for adults aged 60 and over. It is a screening questionnaire that points to whether low mood should be discussed with a doctor — it does not diagnose depression.

What your score means

0-4 — No depression. Your answers do not point to depression right now. Keep up regular activity, contact with people you like, and sleep.

5-8 — Mild depression. Some signs of low mood. Mention this score to your doctor at your next visit, especially if it has lasted more than two weeks.

9-11 — Moderate depression. Clear signs of low mood. Please book an appointment with a doctor soon rather than waiting.

12-15 — Severe depression. Strong signs of low mood. Please see a qualified doctor promptly. If you have any thoughts of harming yourself, seek help the same day.

Frequently asked questions

Is the GDS-15 a diagnosis of depression?

No. It is a screening questionnaire. It indicates whether your mood is worth reviewing; only a qualified doctor can diagnose depression after assessing you in person.

When should I see a doctor?

See a doctor if your score is 9 or above, if low mood has lasted more than two weeks, if you are eating or sleeping poorly, or if you have any thoughts of harming yourself. Thoughts of self-harm need same-day help.

Why is there a separate scale for older adults?

The short form uses plain yes or no questions and focuses on mood and outlook rather than on physical complaints, which in later life often come from other health conditions.

How often should I retake it?

Once every few months is enough unless something changes — a bereavement, a new illness, a move or a change in medication. Retake it sooner if your mood drops.
