CRAFFT Adolescent Substance Use Screen

Youth-focused screen for risky substance use (ages 12–21).

Eligibility: 12–21 years. Enter your age to open the questionnaire.

CRAFFT is a six-item screening questionnaire developed by researchers at Boston Children's Hospital for young people aged 12 to 21. Each item is a yes or no question about risky substance use over the past year. A score of 2 or more is a signal to talk to a doctor, school counsellor or a trusted adult — it is not a diagnosis.

What your score means

0 — No problem indicated. Your answers do not suggest a substance-use concern right now. Keep making safe choices.

1 — Discuss with a trusted adult. One yes answer is worth talking through with a parent, teacher, counsellor or doctor.

2 or more — Concern identified. Two or more yes answers suggest a substance-use concern. Speak with a doctor, school counsellor or trusted adult soon.

Frequently asked questions

Does a high CRAFFT score mean I have a drug or alcohol problem?

No. It is a screening tool, not a diagnosis. It only shows that talking to a trusted adult or doctor is a good next step.

Who can see my answers?

What should I do if I am worried about a friend?
